Communication (or lack of same) is often the most critical aspect of organizational issues.  This may include breakdowns in communication with customers, with employees, with partners, with suppliers, and with fellow managers.  In my experience, there is long-standing and needless waste of human talent and effort that stems largely from failure of communication.
